Tyler was a crew member aboard the F/V Pacific Apollo 50 miles west of St. Paul Island. He was inside a crab pot fixing a wire tie when it was launched accidentally. He was able to get out of the pot, and several attempts at throwing a life ring to him were made. A crewmember was able to reach him but he became listless and slipped out of grip. Despite search efforts, he was never located.
